Endometriosis Saliva Test – Ziwig Endotest®
Endometriosis can cause symptoms ranging from painful periods and persistent pelvic pain to bowel symptoms, pain during sex and difficulty conceiving.
It can also be difficult to identify because symptoms vary considerably between women and may overlap with gastrointestinal, urinary, hormonal and musculoskeletal conditions.
Ziwig Endotest® provides a non-invasive way to support the diagnostic process using a saliva sample. Rather than measuring hormones, the test analyses patterns of microRNAs within saliva.
MicroRNAs are small molecules involved in regulating gene activity – essentially tiny molecules that help control how your genes are switched on and off.
Ziwig Endotest® uses next-generation sequencing (NGS) together with an artificial-intelligence algorithm to identify a molecular signature associated with endometriosis.
The laboratory provides a positive or negative result, which should be considered alongside your symptoms, clinical history and any imaging or specialist assessment.

Who may consider the test?
Ziwig Endotest® may be considered for women aged 18–43 experiencing symptoms suggestive of endometriosis, including:
- Chronic or recurrent pelvic pain
- Painful periods
- Pain during or after sex
- Pelvic pain outside menstruation
- Pain during bowel movements
- Pain when urinating
- Cyclical bowel symptoms
- Blood in the stool or urine during menstruation
- Pain resembling sciatica
- Shoulder-tip pain associated with menstruation
- Difficulty conceiving or infertility

Ziwig Endotest® is intended for women aged 18–43 with symptoms suggestive of endometriosis and is a prescription-only diagnostic test.
A consultation with our clinical team is therefore required before testing to assess suitability.

Diagnostic Analysis:

Analysis of a salivary microRNA signature associated with endometriosis using next-generation sequencing and artificial intelligence.
This technology provides an additional non-invasive diagnostic option for women being investigated for suspected endometriosis.
A French team of experts specialising in endometriosis has recently developed a saliva based diagnostic test that uses Next Generation Sequencing of microRNA’s with AI.

How does Ziwig Endotest® work?
1. Suitability assessment
Ziwig Endotest® must be requested by a doctor.
Your age, symptoms and relevant medical history should therefore be reviewed by our clinical team before testing.
2. Saliva collection
A specialist saliva collection kit is used.
The sample can currently be collected either at our Atlas HQ clinic, or at home according to the instructions supplied with the kit.
3. MicroRNA sequencing
The laboratory analyses the salivary microRNA profile using next-generation sequencing.
4. AI-supported analysis
An artificial-intelligence algorithm evaluates the molecular pattern identified within the sample.
5. Positive or negative result
The result identifies whether the endometriosis-associated molecular signature has been detected.

Clinically Validated Technology:
Ziwig Endotest® has been evaluated in clinical studies involving women with suspected endometriosis.
TDL reports sensitivity above 97% and specificity above 93% within the referenced validation data.
As with all diagnostic testing, results should be interpreted within the appropriate clinical context and test performance may vary between patient populations.

One part of the diagnostic picture:
Ziwig Endotest® can support the diagnosis of endometriosis, but it does not identify:
- Where endometriosis lesions are located
- How extensive the disease may be
- Which organs or tissues are involved
- Ultrasound, MRI and specialist assessment may therefore still be required.
Our pathology guidelines specifically states that Endotest® is not intended to replace pelvic medical imaging, which remains important for lesion description, mapping and classification.
Endotest® provides an additional diagnostic tool within the wider investigation of suspected endometriosis.
It does not identify the location or extent of lesions and does not replace appropriate imaging or specialist assessment.

1. What is Ziwig Endotest®?
Ziwig Endotest® is a non-invasive diagnostic test that analyses a molecular signature of microRNAs within saliva associated with endometriosis.
2. Is Ziwig Endotest® a blood test?
No.
It uses a specialist saliva sample rather than blood.
3. Who is the test suitable for?
The test is currently validated for women aged 18–43 with symptoms or clinical signs suggestive of endometriosis and/or infertility.
4. Do I need a doctor to order the test?
Yes.
Ziwig Endotest® is a prescription-only diagnostic test and must be medically authorised. Your suitability will be reviewed with our clinical team before testing.
5. Can I perform the saliva sample at home?
The samples may be collected in the clinic or at home using the instructions provided.
6. Can I take the test while using the contraceptive pill or other hormonal treatment?
Ziwig’s professional guidance states that testing can be performed while using hormonal treatment.
7. Does it matter where I am in my menstrual cycle?
Ziwig states that Endotest® can be performed at any point in the menstrual cycle.
8. Does Endotest® tell me what stage of endometriosis I have?
No.
It provides a positive or negative molecular result but does not map the location or extent of lesions.
9. Will I still need an ultrasound or MRI?
Potentially.
Imaging remains important for locating and assessing endometriosis lesions, and our pathology provider specifically states that Endotest® does not replace pelvic medical imaging to locate lesions after confirmation of diagnosis.
10. What if I have a negative result but still have symptoms?
A negative result should still be interpreted alongside your symptoms, medical history and other investigations.
Persistent or significant symptoms may warrant further clinical assessment.
NICE guidance also advises that endometriosis should not automatically be excluded solely because examination or ultrasound findings are normal.
11. Can I take the test while pregnant?
No. Ziwig Endotest® is not currently validated during pregnancy.
12. Can I use the test if I have previously had cancer?
The test is currently not validated in patients with a history of cancer.
13. How long do results take?
Our laboratory partner turnaround for the Ziwig Endotest® is approximately 25 working days.
